2026-2028 needs assessments and implementation plans

During 2024–2025, Allina Health partnered with local public health and communities surrounding each of its hospitals to assess health needs, determine priorities and develop a plan to address these priorities for the next three years. Throughout the process, Allina Health Community Benefit and Engagement care team members compared and discussed local learnings to identify common themes. These commonalities resulted in systemwide priorities—i.e., health concerns impacting all communities served by Allina Health—and a shared plan for addressing those concerns from 2026–2028.

Allina Health will pursue three priorities across our system:

  • Mental health and wellness
  • Substance abuse prevention and recovery
  • Health-related social needs and social determinants of health.

Below are links to each hospital’s report and executive summary, which include:

  • A description of the hospital and the demographics of the county surrounding the hospital.
  • A detailed description of the assessment process and results, including local data reviewed, stakeholders who provided input and summaries of key themes.
  • An implementation plan with goals and strategies for addressing these priorities in 2026–2028.
