Community involvement

Allina Health is known for expert treatment of illness and injury. But our service to the community goes far deeper than our clinics and hospitals.

As a not-for-profit health system, we partner and give back to the community in many different ways—all focused on improving the health of those we serve.

Featured initiative

Accountable Health Communities (AHC) model

The Accountable Health Communities (AHC) model is a five-year cooperative agreement between Allina Health and the Centers for Medicare and Medicaid Services (CMS). The purpose of this agreement is to test whether systematically identifying and addressing the health-related social needs of Medicare and Medicaid patients impacts overall health care quality, utilization and costs. Allina Health is one of 29 model participants nationwide and is the only one in Minnesota.

Allina Health began implementing the AHC model on June 5, 2018 at 78 Allina Health clinic and hospital locations across our Minnesota service area. In the first year, we learned unmet health-related social needs impact many Allina Health patients, with food insecurity and housing instability being the greatest needs.
