Jill Konstantinides, PharmD, BCACP
PGY1 Ambulatory Care Site Coordinate

Jill Konstantinides  graduated from the University of Minnesota in 2016. She completed a PGY-1 Pharmacy Practice Residency with an Ambulatory Care Focus and a PGY-2 Ambulatory Care Residency, both at Hennepin Healthcare. Her clinical interests include cardiovascular health, diabetes management, and transitions of care. Jill has been a pharmacist practitioner with Allina Health since 2018. She sees patients at the Shoreview and Bandana Square clinics, as well as does home visits in the north metro region. Jill enjoys the interdisciplinary team approach to care at Allina Health, as well as the focus on population health and care management. Outside of clinic, Jill enjoys hiking, trying new recipes, and spending time with family and friends.

Matt Kresl, PharmD, BCPS

Matt Kresl has worked with Allina Health since 2004. He graduated with his Doctorate of Pharmacy degree from the University of Minnesota in 2004. He completed a pharmacy practice residency at Abbott Northwestern Hospital in 2005. He then worked as an acute care clinical pharmacist and manager at Abbott Northwestern from 2005-2015. He sees patients at the Allina E. Lake St., Bloomington, and Shakopee clinics for patients in the west and south metro region.

Mckaya Kastner, PharmD, BCPS
PGY1 Ambulatory Care Site Coordinator

Mckaya Kastner  is 2013 graduate of North Dakota State University. After graduation, she completed an ambulatory care residency at St. Joseph Regional Medical Center in Mishawaka, Indiana. Mckaya is a Board Certified Pharmacotherapy Specialist and was a pharmacist practitioner with Allina Health for six years. She received her MBA in 2020 and is currently the Director of Pharmacy and Population Health for the Allina Health Group.

Jacob Langness

Jacob Langness, PharmD, BCPS
Abbott Northwester Hospital PGY1 residency program coordinator, pharmacy coordinator for ANGMA General Medicine Clinic

Jacob Langness serves as the PGY1 residency coordinator and preceptor for the PGY1 Internal Medicine rotations and PGY2 infectious disease residency rotation. Jacob earned his doctor of pharmacy from the University Of Minnesota College of Pharmacy in 2010. Upon graduation, he completed his PGY1 residency at University of Colorado Hospital (UCH) and his board certification in pharmacotherapy shortly after. Post-residency, he worked in a variety of clinical settings at UCH, including inpatient clinical pharmacy and outpatient infectious diseases and hepatology. He arrived at Abbott Northwestern in 2016, working in internal medicine, infectious disease, and mental health. He is actively establishing an ambulatory clinical service at our hospital based clinics (ANGMA Medical Clinics) and represents the pharmacy department on multiple department committees including COPD and Falls Prevention. His primary areas of interest are antiviral infectious disease, internal medicine and ambulatory care. He also is the Co-Chair for the Allina Health LBGTQ+ Employee Resource Group and active on the Allina Health Green Committee. In his free time he enjoys chasing his toddler and dog around the park and crafting.
