About Allina & MTM program
Allina Health is a not-for-profit health care system that cares for patients from beginning to end-of-life through its 60+ primary care clinics, 12 hospital campuses, and 14 retail pharmacies. We provide services in the Twin Cities metro area and various greater Minnesota and Wisconsin locations.
Allina Health currently has 15 clinical pharmacists providing Comprehensive Medication Management services to patients. Residents will partner with patients to improve health by providing Comprehensive Medication Management. The clinical pharmacists are present at 27 clinic locations and work under a robust Collaborative Practice Agreement in partnership with providers. Visits occur in primary care clinic offices, over the phone, virtually and in the patient’s home. Allina pharmacists are an integral part of the care management program, which is an interdisciplinary team that includes registered nurses, licensed social workers and non-clinical care guides. The team’s focus is to improve transitions of care and reduce hospital readmissions. Additionally, some of the pharmacists are team members in the Allina Health Senior Health Co-Care Program. This home-based program provides primary care in the homes of geriatric patients.
As Allina’s Medication Therapy Management team has continued to grow and cover a wider area in the Twin Cities metro, we are expanding to offer two PGY-1 Pharmacy Residency Programs. Allina Health – Plymouth Clinic and Allina Health – Hastings Clinic will be the primary practice sites for the programs. Each program will have an additional participating site for patient care, as well as support Senior Health Co-Care.
