To request a copy of your medical records or if you have other release of information questions, call 612-262-2300612-262-2300.
Request a medical record amendment
If there is information in your Allina Health medical record that you believe is inaccurate or incomplete, you have the right to formally request in writing that the information be amended or corrected by completing the Patient Request for Record Amendment form. You may mail the completed form to the address on the form or email the completed form to ComplianceandPrivacy@allina.com. If you choose to email the completed form, please note that unencrypted email is insecure and could be intercepted and viewed by a third party. Allina Health is not responsible for unauthorized access to your health information while in transmission.
To update outdated information, such as your name, contact information, medications or vaccines, please contact your provider or request an update during your next visit.
