C5 integrates 24/7 curbside cardiology consultations, e-consults, coordinated outpatient follow-up, and remote patient monitoring. In just 16 months, the program has facilitated more than 8,000 curbside calls, increased direct admissions by 35%, reduced admission acceptance time by 49% and boosted cardiovascular procedures by 27%.
