During 2021–2022, Allina Health worked with local public health and communities surrounding each of its hospitals to assess community health, determine priorities and develop a plan for how we will address those priorities for the next three years. Four priorities emerged throughout our system: (1) mental health and wellness, (2) substance use prevention and recovery, (3) social determinants of health and health-related social needs, and (4) access to culturally responsive care.
Below are links to each hospital's report and executive summary which include: a description of the hospital and the demographics of the county/communities surrounding the hospital; the detailed process followed to review local data and gather input from a variety of stakeholders and the results of the process; and finally, the implementation plan with our goals, strategies and activities for 2023–2025.
