Employee Occupational Health (EOH) requirements

At the time of your verbal offer you were informed of a 30-minute phone assessment with an employee occupational health nurse. You may also be instructed to visit an Allina Health Laboratory for further testing. Allina Health Laboratory locations and hours.

  • MMR- Mumps, Rubeola (Red Measles), Rubella (German Measles)
  • Varicella – Chicken Pox
  • Influenza (flu) vaccine– Influenza documentation of current season
    •  To seek an exception for this vaccine contact HRConnect Service Center at 612-262-4688 and request the medical or religious exception form.
  • Tdap – required for all new hires that work with and around patients
  • Tuberculosis testing
  • MMR- Mumps, Rubeola (Red Measles), Rubella (German Measles): Accepted documentation includes either 2 documented vaccination dates (after your first birthday) or positive titers showing immunity
  • Varicella - Chicken pox: Accepted documentation includes either 2 documented vaccination dates (after your first birthday) or a positive titer showing immunity
  • Influenza (flu) vaccine: Influenza documentation of current season
  • Tdap: required for all new hires that work with and around patients
  • Tuberculosis testing: Accepted documentation includes either:  1PPD within 12 months of start date or a Tspot/QFT within 12 months of start date.   
    • If you have a history of positive tuberculosis the following should be provided:
      • A copy of your positive tuberculosis testing
      • A copy of negative chest x-ray done after your positive tuberculosis testing
      • Any medical records regarding treatment for tuberculosis
  • Hepatitis B
    • Strongly recommended if you will be working with or around patients. Accepted documentation includes 3 Hepatitis B vaccines or positive titer showing immunity, or you will be required to sign and submit a declination form during your health screen appointment
  • COVID-19 Vaccine
    • COVID-19 vaccination series as indicated by manufacturer

Important Steps to Complete

To formally accept your offer, please complete the following in Workday and via email:

Review Offer letter

You will need to log-in into your Workday candidate account. Your username will be your email and the password you created when you applied. Once you log in you will be able to review and accept your Offer Letter. You will have 24 hours after verbal acceptance to review and accept in Workday.

Background Check

Once you accept the offer, you will be required to review two Background Check documents in Workday. Following that step you will be required to enter your social security number and date of birth.

  • In addition to the Offer letter you will also receive an email from Verified Credentials, LLC with a link to the background form to fill out. You will have 48 hours to complete this form upon receiving the email.
  • Certain positions require a Minnesota Department of Human Services (DHS) background study. Fingerprints and a photograph are required to complete the background study. If the role being offered requires DHS, you will required to review multiple DHS documents in Workday. You will need to take the required action step outside of Workday to complete the DHS background study.
