Allina Certified as Medical Home

[Advance for Nurses, March 30, 2011] With 36 primary care locations certified by the Minnesota Department of Health as health care homes, Allina Medical Clinic is the largest clinic system in the state to receive the designation.

Health care homes, also known as "medical homes," offer a team approach to primary care, making it easier for patients to communicate with their care team. Care teams can include primary care physicians, nurses, specialists, care coordinators and community resources.
