2018 grants awarded

In 2018, grants were awarded to 71 organizations in Minnesota and western Wisconsin to support activities that promote social connections and increased in healthy behavior. Grants ranged from $500 to $10,000. The 2018 Neighborhood Health Connection activities are:

ACTS Big Lake Education on the health value of cultured foods in the diet, demonstration on making these probiotic rich foods and hands on making cultured foods everyone can afford will be provided to participants.
Adaptive Recreational Services (ARS) New Ulm Adaptive Recreational Services participants will meet once a month to create a healthy meal option to take home along with the recipe for further use.  The cooking will be done in groups in order to promote social interaction and cooperation.
Alexandra House, Inc. Blaine Clients of a domestic and sexual violence organization will have the opportunity to participate in twice-monthly Trauma-Informed Yoga sessions held at the shelter. Participants will be provided with yoga mats, blocks, and water bottles.
Braham Area Schools Braham Braham Area School employees will be encouraged to walk/exercise together each Wednesday, eat a healthy breakfast, and attend work on a regular basis.
Breakthrough Ministries, Inc. Burnsville People in recovery from substance abuse will participate in weekly outdoor activities such as hiking, canoeing, kayaking, disc golf, etc. at local and regional parks in the Twin Cities area.
Bridge for Community Life, Inc. Hudson, WI The program will engage people with their community through gardening, cooking and physical fitness.
Bridge on Center New Ulm Cooking Matters is a hands on experience that teaches the basics of cooking.  Participants will learn how to prepare a healthy meal, help a chef prepare the meal and then take home the groceries to prepare the meal for themselves!
Buffalo Community Center Buffalo Residents of Buffalo and surrounding areas will meet once per month to participate in line dancing. Not only does line dancing provide exercise but provides social activity as well.
Buffalo High School Buffalo The Buffalo High School Community will build resiliency skills through happiness practices. Activities of the program include, but are not limited to: community book reads, personal, physical, emotional and social growth groups and activities promoting social connections among various groups.
Buffalo-Hanover Montrose ECFE Buffalo Parental self-care is defined as parental health in the physical. Social, emotional and spiritual areas of daily life. Yoga will be practiced as one strategy in introducing parents of young children to parental self-care.
Cambridge Community Garden (Isanti County Master Gardeners) Cambridge The Cambridge Community Garden provides low cost, easily accessible garden plots to individuals, families and groups.  The Garden will increase the number of raised bed plots for individuals that have a difficult time gardening at ground level.  Social activities will be scheduled to increase interaction and share knowledge about healthy gardening.
Cambridge Isanti Early Childhood Program Cambridge Anoka-Ramsey Cambridge college students, who are also parents, will participate in a monthly and bi-monthly Parent Cafe on campus. A healthy lunch will be provided, along with education and discussions around the areas of stress management, wellness, parenting responsibilities and child development.
Cambridge-Isanti Community Education Cambridge Adults with developmental and/or physical disabilities will participate in a variety of activities including yoga and dancing which can be done from a sitting position, along with walking and ball play.
Cambridge-Isanti High School Cambridge Inter-generational pairs of participants will get a garden site in Cambridge or Isanti, including instruction on how to plant and care for a garden plot, harvest and prepare dishes from the harvest.
City of Cambridge Cambridge Residents in Cambridge will be provided with free outdoor Yoga (Vinyasa and Restorative) classes in Central Green Park.
City of River Falls Parks and Recreation Department River Falls, WI Adults will meet weekly at Glen Park and participate in yoga led by a certified instructor from In Balance.
Community Action Center of Northfield Northfield Participants at Community Action Center Thursday's Table Community Meal will learn how to create healthy meals on a budget. Target population is individuals and families who are experiencing food insecurity or living below the federal poverty line.   
Community And Seniors Together New Ulm Residents of New Ulm, as well as Brown County, will be invited to participate in a free weekly chair exercise class at the Senior Center.
Community Emergency Assistance Programs, Inc. (CEAP) Brooklyn Center Community members will meet at CEAP monthly for healthy living education and participate in monthly health challenges around exercise, healthy eating, mind and body care, and more.
Crisis Nursery Serving Wright County Monticello The Circle of Parents group will take part in wellness and healthy living activities such as yoga, mindfulness and cooking classes with WCCA to introduce families to healthy affordable options in their community.
Delano Senior Center Delano Senior center participants will take part in tai chi classes, with a guest speaker featuring healthy living information.
Emma B Howe YMCA Coon Rapids This activity is designed to build strength, flexibility, range of motion and cardio capacity for individuals with disabilities who participate in our Rise program.
Esperanza - New Creation Lutheran Church Shakopee Children ages 4 - 15 from diverse cultural backgrounds will play recreational soccer Tuesday evenings and Saturday mornings at New Creation Lutheran Church Soccer Field, Shakopee, Minnesota.
Family Pathways North Branch Food shelf participants will receive a Garden -In-A-Box to grow at their home.  Group meetings will be held on how to construct this box, how to grow and care for the vegetables, what healthy eating is about and cook to cook healthier.
Faribault's Future/Faribault Chamber of Commerce Faribault Participants will learn the importance of pollinators and how they affect our food sources through a community pollinator garden.  Participants will also learn how to create their own pollinator garden at home.
First Lutheran Church of Columbia Heights Columbia Heights First Lutheran Church Community Garden is maintained by a core group of workers and fresh produce from the garden is supplied to congregational members, Southern Anoka County Food Shelf, Soup Kitchens and neighborhood members.
Friends of Way Park Northfield Free yoga sessions will be held at Way Park on semi-weekly Saturday mornings open to all.
Friends of Willow River and Kinnickinnic State Parks, Inc. Hudson, WI Women, some of whom may have experienced domestic abuse, will meet monthly at Willow River and Kinnickinnic State Parks. The group will meet ten times for instruction and cooperative skill building on a wide range of activities in the outdoors.
Healthy Community Initiative Northfield Latino community members will participate in a cooking/ wellness club that will take place at Faribault Community School and Main Street Project farm in Northfield. Participants will learn how to cook healthy meals while maintaining their culinary heritage, and reclaim their roots through first-hand experience in vegetable production.
Highland Homes Inc dba Sunset Apartments New Ulm Residents of senior living center will enjoy eight healthy lunches and 24 weeks of strengthening exercises.
ICA Food Shelf (on behalf of the Blake Road Corridor Collaborative) Minnetonka Neighborhood residents and community members will have opportunities to join cooking gatherings, yoga, bike activities, and gardening activities in Hopkins.
In His Steps Ballet & Performing Arts Company Cambridge The activity assists victims of domestic and/or sexual violence in achieving physical and emotional healing. Participants will attend group art and barre fitness classes.
Industries, Inc. division of PHASE Cambridge Adults with disabilities and the staff that support them will participate in a Wellness Program that will include physical activity, nutrition education, and stress reduction.  The majority of activities will take place in community integrated settings. 
ISD 882 Monticello Multiple generations will come together to create raised-bed gardens.
Jefferson Elementary School New Ulm Staff from Jefferson Elementary School will be invited to participate in 'Chaos to Calm', a program of six sessions (held at the Wellness Collective in New Ulm) designed to reduce stress and build resiliency. Self-care sessions will include yoga, breathing techniques, stress awareness, healthy eating habits, stress reduction and resiliency.
Keystone Community Services St. Paul Seniors will participate in evidence based classes to improve their health. Staff and volunteers will become trained facilitators to teach classes within our senior center.
Liberian Health Initiative Minneapolis Immigrant women and men will use cultural health beliefs to learn about the risks of hypertension, diabetes and mental health through nutrition, exercise, and treatment compliance.
Mile in My Shoes Minneapolis By bringing the power of running to people experiencing homelessness, our teams build bonds, boost health and wellness, inspire one another, and spark social action. We do this through weekly morning runs, monthly road races, and regular social and educational events. Team Volunteers of America-Roseville will bring together community runners with men and women re-entering civilian life after long-term sentences of incarceration.
Monticello Community Center Monticello Seniors will be invited to attend gentle Tai Chi using the Yang 10 form of Tai Chi at the Monticello Community Center.
MRCI Worksource (EASE) Adult Day Service New Ulm Participants at the MRCI- EASE Program will be presented with a fact about one healthy food item each month and how to prepare the healthy foods into an easy healthy snack.
Native American Community Clinic Minneapolis Grow Your Own curriculum will be provided through 18 weekly classes at a community garden, Mashkiikii Gitigan. Class topics include nutrition, native health & wellness, medicinal herbs & teas, wild foraging, traditional tobacco, seed keeping, lead awareness, and gardening basics.
New Ulm Park & Recreation New Ulm Free Mobile Recreation throughout Community Parks and Private locations will be offering a variety of different activities to all ages such as arts & crafts, athletics, games, and more. 
Northfield Women's Center Northfield Low income single moms and families will have the opportunity to participate in the Cooking Matters program offered by the U of M Extension Program. In addition to 6 cooking classes we will have 3 fitness lessons while providing childcare for all 9 sessions.
One Yoga Minneapolis Women at Minnesota Correctional Facility - Shakopee will participate in a two hour weekly class on yoga and mindfulness.
Polaris Battalion United States Naval Sea Cadets Isanti This activity is for the Adult volunteers from Polaris Battalion and includes a walking program, weight loss challenge and monthly meals together.
Prescott Public Library Prescott, WI Beginner Adult Yoga and Nutrition Classes will be offered at the Prescott Public Library. These classes will be offered so that our community can have access to healthy living without the cost. Adaptive yoga for disabled or senior citizens will also be offered.
Pro Kinship for Kids New Ulm Families of Brown County will be learning about healthy eating and playing games as a family to learn about physical activity at New Ulm Mobile Village on a monthly basis.
Rice County Friendship House Faribault Adults with Serious and Persistent Mental Illness will participate in hands-on gardening classes and cooking classes at the Friendship House, a Resource Center/Clubhouse in Faribault, MN. Participants will then take home groceries in order to duplicate the recipe at home. The Friendship House is collaborating with the University of Minnesota Extension Office to provide the  cooking classes.
River Falls Housing Authority River Falls, WI Low income, elderly and disabled tenants will be given the opportunity to participate in a walking club, a balance and fall prevention program, healthy eating on a budget and cope with mental illness.
Ruth's House of Hope Faribault Residents of Ruth's House will participate in Healthy Cooking Classes, Mindfulness/Meditation, swimming, and activities at River Bend Nature Center.
Scott Carver Dakota CAP Agency Shakopee Parents Helping Parents summer activities are designed to help parents get their families out in the community, connecting with others and participating in activities that promote a healthy lifestyle like gardening, yoga and hiking . Our activities will take place at local parks throughout Scott and Carver counties.
Scott County Human Services Adult Mental Health Shakopee Individuals experiencing severe mental illnesses will undergo health screenings to determine their current health status and then begin participating in a variety of outdoor physical activities in team competitions throughout the summer.  In addition, they will sample healthy food choices and be provided opportunities to use these products at home.
Senior Center Board Coalition of St Croix County over Hammond Nutrition Site Hammond, WI The Second Saturday of the month we will sponsor a healthy lunch with an activity such as yoga, tai chi and Zumba and a social hour after.
Senior Community Services Minnetonka The “Living Wellness” program will be led by a certified wellness coach and personal trainer who will help older adults formulate goals and strategies for exercise and better nutrition.  The program will include topics on nutrition and wellness which will take place at the Monticello Senior Center and a local grocery store tour. 
Shakopee Diversity Alliance Shakopee Participants will receive an eight week membership and will be trained how to reduce stress with breathing. We will teach mindfulness and self-care skills, followed by a discussion.
Somali Community Resettlement Services Faribault A holistic approach to wellness, addressing mind, body and spirit, will be taught to area Somali immigrants through weekly small group sessions. The sessions consist of circle time, education and learning, cooking, review and goal setting.
Spirit River Foundation Isanti Spirit River Community Church will host "Healthy Lifestyle--Body, Mind, Spirit," a three-hour six week program targeting adults living with mental health challenges.  The participants will combine hands-on-food preparation and nutrition information, practice mindful breathing, and become aware of how stress influences their emotions and health behavior.
Spring Valley Seniors Staying Put, Inc. Spring Valley, WI We will be “Expanding Horizons” for Spring Valley Seniors Staying Put, Inc. participants in Spring Valley by offering a wide range of healthy activities.  Examples are: a greenhouse visit to investigate growing healthy food, and an expert led geology tour and many walk and talk sessions. 
St. Mary's Health Clinics St. Paul Latino women living in the Phillips Neighborhood of Minneapolis will create community and healthy connections through the organization and participation in free Zumba classes, nutrition presentations, community garden and family celebrations.
Stepping Stone Anoka Residents of Stepping Stone Emergency Housing will participate in a monthly adventure club to build social skills while participating in healthy physical activities. Residents will incorporate the state parks for snowshoeing, canoeing and hiking.  Residents will go bowling and visit an adventure park during the bitter cold months.
The Lodge of Winthrop Winthrop The residents of the Lodge of Winthrop (assisted living) and GSS- Winthrop (skilled care facility) will enjoy "Mindful Melodies Music Therapy". Bi- monthly sessions will be held with a Theme each session in the dining room of the GSS- Winthrop.  There will be reminiscing, singing and socialization.
The Open Door Eagan The Garden to Table program will support adult food shelf clients with their gardening endeavors. Various classes throughout the calendar year will be held to support client gardeners, encourage connectedness and build and strengthen relationships amongst gardeners.
Timber Bay/Youth Investment Hamel Young adults from difficult backgrounds will begin to realize the benefits of healthier living through keeping an exercise and nutrition log book — and discovering the joy of healthy exercise.
Turningpoint for Victims of Domestic and Sexual Violence River Falls, WI The program will offer 5K and half-marathon training programs for women victims/survivors of domestic and sexual violence. The training programs will begin in May and conclude on September 29, 2018 with Turningpoint's Empowered Women's Half-Marathon and Women's 5k events in River Falls.
Vital Aging Network North St. Paul Phillips Wellness 50+ encourages older adults, many at risk for health disparities, to take actions to improve their health and well-being and support others in doing the same. The resident-powered initiative organizes weekly gatherings for social connection, a group walk, Tai Chi, learning about diabetes management and more.
Volunteers of America Adult Day Services Sleep Eye Participants of Adult Day Services and Residents of Ross Park Apartments, all of whom are low income seniors, will participant in a 5 month exercise program led by the Live 2B Healthy company. This program is targeted to increase balance and strength and help prevent falls.
Woodbury Community Foundation Woodbury Woodbury’s Inclusive Health and Fitness Program is designed for adults with intellectual disabilities and those without disabilities.  We will partner with Woodbury Thrives, Woodbury YMCA, Ways to Wellness and others to offer programs that allow people with intellectual disabilities to be seen as equal in the eyes of the community. 
YMCA Great Twin Cities Lino Lakes Lino Lakes Residents of Centennial Square will be provided with nutrition, strength, and cardio classes and will meet together twice a week for social activities.
YMCA of the Greater Twin Cities Minneapolis The Downtown St. Paul YMCA will partner with Ramsey Women's Correctional Facility to offer female offenders weekly seminars focused on healthy eating, physical fitness, and stress management. Seminars will be held at the facility and focus on helping women increase both short-term and long-term whole being wellness.
Zion Lutheran Church (on behalf of Buffalo Area Dementia Friends) Buffalo Buffalo Area Dementia Friends Caregiver support group meets weekly.  Each Caregiver participant is supported as they share any challenges they may have concerning their loved one with dementia.  We are encouraging healthy living through diet and exercise.
