Assessment

Your initial appointment includes a 40-minute consultation with a preventive cardiologist. A thorough cardiovascular history and physical examination will be performed, with assessment of your medical health, family history, and lifestyle factors that impact your risk of heart disease. We provide guidance on how to optimize your heart health, and we partner with you in how to preserve or improve heart health.

The tools we may use include:

  • Lipid Clinic and advanced cholesterol testing
  • Coronary Calcium Score – screening test for coronary artery disease
  • Abdomen aortic aneurysm (AAA) screening
  • Heart Healthy Lifestyle Program” - see below!
  • Dietician consultation
  • Stress reduction
  • Mindfulness Training
  • ExerCare Fitness Center Membership

The time:  

We also offer a “Heart Healthy Lifestyle Program,”which is designed to help you make lifestyle changes to reduce your risk of heart disease. This three-month program involves frequent in-person visits and telephone check-in calls to help you stay on track with developing a pattern of healthy eating and regular physical activity. Change takes time and effort, and we are here to support you on this path!  

Visit 1:  60 minute consultation with Registered Dietician and 60 minute consultation with Exercise Physiologist 
2 weeks later:  telephone check-in
2 weeks later:  40-minute clinic visit with Registered Dietitian or Prevention Nurse Practitioner
2 weeks later:  telephone check-in
2 weeks later:  40-minute clinic visit with Prevention Nurse Practitioner
4 weeks later:  telephone check-in or in-person visit with Exercise Physiologist
6 weeks later:  telephone check-in
8 weeks later:  30-minute appointment with Exercise physiologist and 40-minute appointment with Prevention Nurse Practitioner or Cardiologist

At this time, the visits with our exercise physiologist and telephone-check in visits are offered at no cost to you! The remaining visits with our dietitian, nurse practitioners, or physicians are billed through health insurance plans, or there is a set out-of-pocket cost if there isn't adequate coverage.
