Coronary artery disease (the most common form of heart disease) is the leading cause of death for both women and men in the United States.

It is estimated that coronary artery disease claims over 500,000 lives each year—more than all forms of cancer combined!

But heart disease doesn't just result in death—it can damage your heart and reduce your quality of life. That's why it is so important to know the risk factors and early warning signs of heart disease.

The key to fighting heart disease is PREVENTION.

What is Prevention Clinic?

This unique program combines the expertise of preventive cardiologists, registered dietitians, exercise physiologists and nurse practitioners to assess your heart health and risk for heart disease.

We have the time, the tools, and the team to help you realize the health you've imagined.

The Time

You will start with a 40-minute consultation with a preventive cardiologist. A thorough cardiovascular history and physical examination will be performed, with assessment of your medical health, family history, and lifestyle factors that impact your risk of heart disease. Based on your individual needs, your cardiologist will recommend a variety of tools to optimize your heart health.

 

Heart Healthy Lifestyle Program

After meeting with your preventive cardiologist, you may be referred to the Heart Healthy Lifestyle Program—a three-month program consisting of in-person and telephone check-ins designed to help you make lasting lifestyle changes to reduce your risk of heart disease. 

The program outline:


Visit 1

  • 60-minute consultation with an Exercise Physiologist at ExerCare Fitness Center
  • 60-minute consultation with a Registered Dietitian

Visit 2 (two weeks after Visit 1)

  • Telephone check-in

Visit 3 (two weeks after Visit 2) 

  • 30-minute visit with the Exercise Physiologist at ExerCare
  • 40 minute follow up with the Registered Dietitian

Visit 4 (two weeks after Visit 3)

  • Telephone check-in

Visit 5 (two weeks after Visit 4) 

  • Telephone check-in 

Visit 6 (two weeks after Visit 5) 

  • Telephone check-in

Visit 7 (two weeks after Visit 6) 

  • 30-minute appointment with Exercise Physiologist at ExerCare

At this time, the visits with our exercise physiologist and telephone-check in visits are offered at no cost to you!

Additional visits can be made with our dietitian depending on individual needs. These visits are billed through health insurance plans, or there is a set out-of-pocket cost if there isn't adequate coverage.

  • Lipid Clinic and advanced cholesterol testing
  • Coronary Calcium Score: screening test for coronary artery disease
  • Abdomen aortic aneurysm (AAA) screening
  • Dietitian consultation
  • Stress reduction
  • Mindfulness Training
  • ExerCare Fitness Center Membership

  • History of heart attack, stents or coronary artery bypass surgery
  • Coronary artery plaque or calcification found on an imaging test, such as a CT scan
  • Family history of heart disease
