Comprehensive medication review

  • Pharmacists have an important role in your care

Taking prescription medicines can be a challenge. There are a lot of details to understand. One member of your health care team - a pharmacist practitioner - can help you manage your medicines and your health. A pharmacist practitioner is doctor of pharmacy (PharmD) that is specially trained to provide patient care in the clinic setting.

Several Allina Health clinic locations offer a special clinic visit that lets you talk privately with a pharmacist practitioner about your medicines.

During the review, the pharmacist will:

    • tell how safe your medicines are
    • tell how well your medicines are working
    • assess any needs or problems caused by your medicines
    • create a care plan just for you
    • give suggestions to your regular health care provider
    • schedule a follow-up appointment (if needed)
    • answer your questions.

Bring all of the medications you are taking. Include prescription, over-the-counter, vitamins, herbals and natural medicines. The pharmacist practitioner needs to know all of the medicines you are taking.

Also, bring a list of questions. For example, do you know what to do if you forget to take your diabetes pills, is there an herbal you want to start taking, or do you think your asthma inhaler isn't working as well as it used to.
