  • Minnesota Senior Health Options care managers

    Our registered nurse and social worker care managers help seniors in Minnesota Senior Health Options (MSHO) understand and manage their health care and arrange related support services. MSHO is for people ages 65 and older who are eligible for Medical Assistance and are in Medicare Parts A and B or who have MA only.

  • Aging with independence.

    The SeniorCare Transitions team includes a nurse practitioner and physician who will travel to you and provide primary care in the comfort of your own home. This convenient health care option means that you no longer have to travel to appointments and spend time in waiting rooms. We make house calls to partnering senior living communities on weekdays and can be called 24 hours a day, 7 days a week.

    Our goal is to help you stay healthy, living as independently as possible. The SeniorCare Transitions team is made up of geriatric medicine specialists who know how the aging process affects the prevention, diagnosis, treatment and management of conditions that are common to the elderly. We will take the time to learn about you, your needs and goals and help you manage your medical conditions, care for you when you are sick and need medical care.
