SeniorCare Transitions offers medical care services to patients who need a short-term stay (usually two weeks or less) and rehabilitation as they continue to recover from a hospital stay.
Our goal is to help you make a safe transition to home or other suitable living arrangement.Your SeniorCare Transitions medical team will come to the transitional care unit where you're staying to: manage your medical care, rehabilitation and progress after your hospital staycoordinate your care using Allina Health electronic medical recordshelp you overcome health barriers to provide a safe transition homecommunicate with your primary care doctor and other home services to make sure you get the care you need when you leave the transitional care unit.
Your SeniorCare Transitions medical team will come to the transitional care unit where you're staying to:
An experienced nurse practitioner and a physician will make onsite visits to the TCU. You will normally be seen one to two times per week. SeniorCare Transitions teams are on-call 24 hours a day, 7 days a week.
Our medical teams provide care in a wide network of transitional care units in the extended Twin Cities metropolitan area.
