    A unique outlook on primary medical care

    The Advanced Primary Care Clinic is a medical home for people with disabilities or other complex health conditions. The clinic offers medical, rehabilitation and psychiatric care in one accessible clinic location. It's a new way to experience primary medical care focused on keeping you as healthy and independent as possible.

    The doctors, nurses and nurse practitioners partner with patients and families to set goals that meet individual needs. Early intervention techniques - including same-day or next-day appointments - are used to reduce hospital stays. Care is coordinated within the clinic and with outside providers.

    The Advanced Primary Care Clinic provides:

    • Preventive and wellness care
    • Pain management
    • Management of prescription refills
    • Scheduling of X rays and lab work
    • Ongoing nurse support and follow-up
    • A nurse line to call for questions
    • Telemedicine monitoring visits
    • Sub-specialists such as care coordinators and dietitians
    • Social worker consultations
    • Referrals to additional resources

    In joining our medical home you are responsible for:
