Allina Health has partnered with Honoring Choices Minnesota to implement a local program which focuses on end of life choices and promotes awareness and participation in advance care planning (ACP). ACP helps people think about, talk about and document choices for future health care decisions and living life to its fullest. The written plan, or health care directive, makes wishes known to health care providers and family members if a time comes when injury or illness prevents someone from being able to speak for themselves.
Most people have not engaged in family conversations nor have they documented their life choices. The Honoring Choices program helps people understand end of life choices and the implications upon themselves, their families and others close to them. Jointly funded by the Faribault Area Hospice Foundation and the Owatonna Hospital Foundation for three years, the program will be led by an advance care planning coordinator.
The program is seeking applicants for the coordinator position and volunteers to help facilitate classes and events.
