Surgery checklist

    • Schedule your history and physical (H & P) exam.
    • Make a copy of your health care directive (or living will) form.
    • Arrange for someone to drive you home. If you arrive for your surgical procedure without someone to accompany you home your surgery may have to be cancelled and rescheduled.
    • Communicate your special needs.
    • Avoid smoking, street drugs or alcohol.
    • Complete any preparations as instructed by your doctor.
    • Do not eat or drink the day of your surgery. 
    • Take your physician-approved medications or pain pills as arranged in advance.
    • Remove all jewelry (including wedding rings and body piercings) and contact lenses. Do not bring these items to the hospital.
    • Bring these items to the hospital:
      • container for glasses, dentures, or hearing aids
      • copy of health care declaration (if staying in the hospital)
      • toiletries
      • reports from doctor
      • insurance card and Social Security number
      • no valuables
