Care coordination

  • A brain tumor diagnosis can feel overwhelming for the patient and family alike. And navigating a hospital or health care system isn’t always easy under the best of circumstances. The Givens Brain Tumor Center understands how important it is to have someone who understands the system to help guide you through the process.  We offer neuro-oncology care coordinators and a LifeCourse care guide to support patients and their loved ones from the time of diagnosis throughout their care journey.

    What is a neuro-oncology care coordinator?

    Care coordinators are experienced registered nurses who provide critical support to patients and their families. They are available to the patient and their family from diagnosis through treatment and on to healing. It is their goal to ensure that you understand every step of this process and receive the best care possible. 

    The role of the neuro-oncology care coordinator includes:

    • working closely with various doctors’ offices to speed up scheduling of appointments
    • ensuring that appropriate work-up and staging of a patient’s disease are completed in a timely manner
    • providing ongoing support and education to patients and their caregivers
    • educating patients about available support programs and services, including integrative health therapies and palliative care services
    • follow-up treatment coordination and survivorship visits, including cancer rehabilitation referrals.

    What is a LifeCourse care guide?

    The Givens Brain Tumor Center has a dedicated LifeCourse care guide. The LifeCourse care guide is a lay health care worker role within Allina Health. Care guides receive training in a family-oriented approach that helps patients live well with a serious illness and supports their care circle. He or she acts as a key communicator across health care and community settings to support patients and their care circle in seeking care that aligns with the patients’ goals and preferences.
