Care coordinators are experienced registered nurses who provide
critical support to patients and their families. They are available to the
patient and their family from diagnosis through treatment and on to healing. It
is their goal to ensure that you understand every step of this process and receive
the best care possible.

The LifeCourse care guide is a lay health care worker role within Allina
Health. Care guides receive training in a family-oriented approach that helps
patients live well with a serious illness and supports their care circle. He or
she acts as a key communicator across health care and community settings to support
patients and their care circle in seeking care that aligns with the patients’
goals and preferences.
