Home health, palliative and hospice care

River Falls Area Hospital will help coordinate referrals to agencies that provide comfort, support and compassionate care for patients needing rehabilitation, advanced illness or end-of-life care and the loved ones caring for them.

Our mission is to help honor the wishes of patients and families and in doing so bring peace, dignity and comfort 24 hours a day, seven days a week.

Patients have a choice as to which agency they use. However, choices may be limited by the breadth of services offered, insurance coverage and agency availability.

• Home health care is appropriate for persons who prefer to stay in their home but are in need of ongoing skilled care that cannot be adequately provided by themselves, their family or friends.
• Palliative care services are for patients and their families facing the problems associated with an advanced, progressive, chronic or incurable disease, yet who may be receiving aggressive treatment for their illness.
• Hospice care is for anyone with a life-limiting illness and whose life expectancy is determined to be six months or less if the disease runs it natural course.

Adoray Home Health and Hospice

Adoray Home Health and Hospice is a Medicare-certified agency that is partially owned by River Falls Area Hospital. Adoray works with patients and families to help determine eligibility for services and to verify insurance coverage.

Home health care services

A team of home care professionals, with direction from your physician, carefully plans and provides the services required to meet your comprehensive health care needs.

Palliative care services

According to your needs and wishes, Adoray will assist in the prevention and relief of physical symptoms, as well as provide emotional and spiritual support. Usually you must be considered homebound for palliative care services.

Hospice care services

Adoray's interdisciplinary hospice team focuses on enhancing quality of life wherever terminally ill patients call home, through expert pain and symptom care along with emotional, social and spiritual support.

Adoray's experienced and dedicated registered nurses, aides, social workers, chaplains, physical therapists, occupational therapists and other professionals and volunteers will work to develop a flexible, personalized care plan for you and your family. The hospice plan of care may include medical supplies, equipment and medications related to the life-limiting illness. Short-term respite care and bereavement support are also available.
