The Community Falls Prevention Program is focused on helping seniors stay healthy, safe and independent for as long as possible.

This initiative is led by River Falls Area Hospital in partnership with the physicians at the River Falls, Ellsworth and Spring Valley medical clinics.

The Community Falls Prevention Program helps community members 65 and older identify risks that can lead to falls.

The community falls team will provide one-on-one consultations, recommendations and education to greatly decrease risk factors for falls.

A nurse navigator will be your advocate and resource to ensure any needed assessments and care is coordinated.

Together, you will work to help eliminate your fall risks – ensuring your continued independence for as long as possible.

  • Devastating: One in ten falls in older adults results in serious injury and loss of mobility and independence. Two-thirds of those who fall will fall again.
  • Deadly: Falls are the leading cause of death in older adults.
  • Preventable: Reducing identified risks of falls will prevent falls and lead to better, longer lives.
