The Community Reintegration Program (CRP) is an intensive outpatient group treatment program designed to maximize the abilities and independence of those who have sustained mild-to-moderate brain injuries. Other diagnoses which affect cognition, and significantly impact functional independence may also be appropriate.
Individuals typically attend the program three days per week, at least four hours per day, for about six months. Participants establish individual goals while also working in a group setting to receive support, gain independence and improve their pre-vocational skills.
Other services, such as speech therapy, occupational therapy or psychotherapy can be added as needed.
The Community Reintegration Program is a medically prescribed service. Fees are billed to third-party payers, such as Medical Assistance, Special Needs Basic Care (SNBC) plans, Worker's Compensation and some private insurance plans. Medical Assistance requires a neuropsychological evaluation within six months of an individual starting CRP. Neuropsychological evaluations can be scheduled at Courage Kenny Rehabilitation Institute-Golden Valley.
For more information or to make a referral, call Jennifer Kahn at 612-775-2629; faxed orders for CRP evaluations can be sent to 612-262-6709.
The Community Reintegration Program is based at the Courage Kenny Rehabilitation Institute's Golden Valley campus.
